Amenities
Your comfort is important to us!  For your convenience we provide pillows, blankets, satellite radio and cable television to help you relax during the procedure.  Nitrous oxide is also available.

We strive to stay abreast of the latest in technological advances and methods that will give you better results in less treatment time.  Dr. Sporer and Dr. Cutler perform all procedures with the best equipment available today.

Appointments
We know your schedule is busy! For your convenience, early morning and lunchtime appointments are available.  Your scheduled appointment time has been reserved specifically for you.  We request 24-hours notice if you need to cancel your appointment.  We are aware that unforeseen events sometimes require missing an appointment.  After missing your second appointment without notifying us 24 hours in advance, you are subject to being charged an additional fee.  Same-day appointments are available for emergencies.

Your First Visit
Please arrive 10-15 minutes prior to your appointment time to fill out a general information and medical history form. If your initial visit is for a cleaning and examination , your visit will be for approximately one hour in length. During that hour you will receive a cleaning, all necessary x-rays and an oral cancer screening by our hygienist. Next, your doctor will perform a thorough oral examination. If any further treatment is necessary, you will receive a treatment plan and be schedule accordinly. If you have any medical conditions that would require you to pre-medicate, please inform our scheduling coordinators when making your appointment. If you have previous records you would like us to have, please advise your schedule coordinator.
If you would like, Please download the Health History form (from our "Office Forms" list), complete it and bring it with you to your appointment.

Insurance

We believe you deserve the best care.  That's why we always present you with the best dental solution possible to treat your personal situation.  Each year we provide outstanding care to hundreds of people.  Some have dental benefits, but most don't.  If you have dental benefits, congratulations!  You are very fortunate.  Here are some important things you should know.  Insurance companies do not recognize many routine and newer dental services.  Implant services and Bruxism appliances are prime examples.  Your dental benefits are based upon a contract made between your employer and an insurance company.  Your range of benefits depends solely on what your employer decided to purchase.  Some plans cover as little as 30% or as much as 100% of dental services (less applicable deductibles, and plan maximums).  Some plans base the amount of benefit on a schedule of fees arbitrarily developed by insurance companies.  For this reason, you may receive a lower percentage than the reimbursement level indicated in your dental plan. Your insurance company is responsible to you, and not to our office. However, we will assist you in any way we can to maximize your available benefits.  Please remember, dental benefits will rarely pay for the completion of your entire dental care.  It is only meant to assist you.  If you have any questions regarding your dental benefits that we can not answer please contact your employer or insurance company directly.  We will do our best to help coordinate your dental benefits so that you receive the maximum possible reimbursement for the services we have provided you.  As a courtesy, our staff will gladly provide you with any necessary forms or submit any paperwork on your behalf to assist you in receiving direct reimbursement by your insurance carrier.  If your carrier accepts electronic claims we will file for you.

Financial Policy
Please ask a member of our staff about financing options available.  We will gladly review our payment options to accommodate your financial needs.  If special arrangements are needed, please talk to our office manager prior to receiving service.  Your treatment plan will include a breakdown of all applicable fees, and we will inform you of all costs before treatment is administered.  Charges are payable at the time of treatment by either cash or check.  In addition Visa, MasterCard and American Express are also accepted. For patients whose treatment will be completed over several visits or months, we offer 1/3 payments. The first 1/3 is due at the first treatment visit.  The second third in the middle of treatment and the last third near the end of treatment.  To allow us to save on accounting costs, we offer our patients a pre-payment savings if they pay their treatment plan in full at least one week prior to their first appointment to begin the treatment. This offer applies ONLY to any treatment plans over $7,500.00. A 10% accounting reduction is offered if pre-paid in full by check or cash, or 5% by Visa, MasterCard, or American Express. 0% Financing over 12 Months or Long Term Financing:  We work with CareCredit, a third party financer, which offers our patients a low interest rate and a low monthly payment financing. Please ask for an application. (0% financing available over 12 months or extended payment plans available at about 11.9% interest.)  You can also apply online at: www.carecredit.com.
